WELCOME PAWS: KOREAN DOG RESCUE Adoption Application

This questionnaire is designed to help Welcome Paws and you determine if a specific dog is the right one for you and your household. All adopting parties are required to complete this form prior to adoption. Welcome Paws reserves the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions!

When applying, we ask for 100% commitment from all your family members. Our dogs require time, love and patience. Our dogs are assessed in a shelter environment while in our care, however we cannot guarantee behavior in a new environment with a new family. 

Please only apply if you and your entire family are committed to providing these dogs with what is required.

Adoption Fee USD Varies from $600-$900. This will include, 1. Airfare 2. All vaccinations, spay & neuter, rabies, parasite prevention treatment 3. Health examination 4. Airline approved kennel 5. Leash and collar or Harness 6. Veterinarian inspection certificate 7. Korean quarantine screening fees 8. Microchip 9.USDA import permit (US only) and Custom's entry document 10. Airport transportation. ***FULL PAYMENT IS REQUIRED TO FINALIZE THE ADOPTION***
